1. Standard Ambulatory Service
For patients who are able to stand, step into a vehicle, or use lightweight walking aids.
Base Rate / Pickup Fee: $35.00
Mileage Rate: $2.75 per loaded mile
Includes: Door-through-door assistance, escorting to/from check-in desks, and accommodation for folding walkers or canes.
2. Wheelchair & Specialized Mobility Service
For patients requiring hydraulic ramp loading and full wheelchair tie-down securing.
Base Rate / Pickup Fee: $55.00
Mileage Rate: $3.25 per loaded mile
Includes: Use of specialized hydraulic ramp, 4-point ADA-compliant wheelchair restraints, and nurse-led transfer assistance.
3. Flat Local Hospital Discharge Rate
A simple, predictable option for local hospital-to-home or hospital-to-rehab discharges.
Local Discharge Package (Within 15 Miles):
Ambulatory / Walker: $75.00 Flat Rate
Wheelchair: $105.00 Flat Rate
(Distance beyond 15 miles is billed at standard per-mile rates above).
4. Recurring Dialysis & Rehab Package Rates
Discounted rates for predictable, 3x/week scheduled appointments.
Dialysis Round-Trip Base Rate: 10% discount off standard base rates for contracted recurring schedules.
Guaranteed Pickup Window: Prioritized schedule placement to ensure zero missed treatment sessions.
